PRESCRIPTION INFO: RX order form included: You'll need to supply your current prescription and indicate the lens tint that you want your RX in; Clear, Gray or Amber - then you'd send in the order form along with the information needed. There is a charge of $89.00 for the polycarbonate lens or you can get two tints for $160.00. This is done through the manufacturer.
